VA text orders


Below are some common text orders that I've saved (just to save time from typing it out for every patient). You can use the tabs above for some more interactive text orders (that change depending on the input)

Plain text orders

Wean O2

Please wean supplemental oxygen Q2H for goal SpO2 between 92-97% (e.g. SpO2 >97%, decrease supplemental O2). If SpO2 >92% on 1L NC, turn off supplemental O2

Strict I/Os

STRICT I/Os: Please document strict intake and output per shift. May place condom cath or purewick if needed. If unable to record strict I/Os for some reason, please contact the inpatient provider.

MRSA Screen

Please actually collect the MRSA screen (nasal swab) that was automatically ordered on admission

Vanc trough

Please *ward collect* vancomycin trough 30 minutes before next vancomycin level is due

Stop the accuchecks

Please stop checking routine accuchecks/POC glucose. May check POC glucose concerned for overt hypoglycemia.


This can be used to order bladder scans / post void residuals (PVR) and gives instructions on what the nurse should do. It tells them to check PVRs at a certain frequency. If given a cutoff for the bladder scan, it tells them to call the team &/or straight cath the patient. Most importantly, it gives them instructions about documenting the results, so folks can find it later on.

It's always a pain when some of the patient's are listed under the wrong inpatient provider, and this happens alot on RITE team. This generates a text order to change the inpatient provider (&/or attending) depending on the names that are entered in the boxes below. You can use the drop down menus to specify if this is a change (i.e. provider to XYZ, keep attending as ABC).

Should also mention that heparin kills people

In additon to placing the text order, you must: (1) verbally communicate the plan with the bedside RN, (2) place the order for the repeat PTT, and (3) order the heparin bolus, if applicable
