How to make new templates

Below is a quick guide to creating new templates in CPRS. In the example below, I use the following template:

============================= INPATIENT MEDS =============================

|ACTIVE MEDICATIONS|

============================= OUTPATIENT MEDS ============================

|OUTPATIENT MEDS|

...
Figure 1: Open the panel

First go to the Notes tab of CPRS. In the bottom left hand corner, click on Templates to open the templates panel (Figure 1)

Next, right click on My Templates and select Create New Template (Figure 2, below)

...
Figure 2: Open the window to make a new template

This will bring up a new window (Figure 3)

  • First, you need to give the template a name (Step 1, in red)
  • In the large text box below you'll enter the template text (Step 2, in blue)
  • To save, hit Apply or OK (Step 3, in green)

TIP: You can preview the output of the template from this screen with the shortcut ctrl + T while this screen is open

Behind the scenes, the text that is surrounded by vertical bars (e.g. |ACTIVE MEDICATIONS| and |OUTPATIENT MEDS|) are called patient data objects, and work similar to Smartlinks in Epic. You can expore these patient data objects in CPRS with the shortcut ctrl + I (but it's a very long list).

...
Figure 3: Creating the template

If you close out of the window from the last step and use the templates panel (Figure 1), you should see your new template listed under "My Templates"

To preview the template, *single* click on the name of the template (red arrow in Figure 4). Pressing ctrl + W with the template selected will bring up the template with autopopulated text, as shown in Figure 4

To insert the template into a note, you need to have an unsigned note that you're editing. If you **double** click on the name of the template while editing your note, the template will insert at whatever point you're typing in the note

...
Figure 4: Previewing the template using ctrl + W

Other templates I use

What does [___this___] mean?

You'll see lots of places in my templates that have [____] or [___text___]. This is because I use Dragon for my notes and it allows me to use shortcuts to skip through these placeholders (similar to using F2 and the *** in Epic). Since this isn't anything specific to CPRS, you can remove these, make them ***'s, or do whatever you like

NOTE: this is not to be confused with the vertical bars ( | ), which are patient data objects. See Step 2 above for details

Why the U?

There are also (seemingly) random U's in my notes. These prevent you from signing notes (a standalone U is an abbreviation that isn't allowed by CPRS), so it keeps me from accidentally signing the note before I finish it. This is similar to the *** used in Epic

INPATIENT TEMPLATES

This is my admission H&P. Be sure to replace the parts in red to reflect your name / team / contact info

AHCP

Similar to above, this is just a plain text template I use for preparing the "After Hospital Care Plan". I normally do this in a word doc, since the AHCP likes to crash

====================== Reason for Hospitalization =======================
You were admitted to the hospital because

========== Symptoms to Watch For After Patient Leaves Hospital ===========
Please seek medical attention if you are experiencing symptoms like the
ones you had prior to coming to the hospital.

We want to ensure that you have the appropriate help after you have been
discharged. PLEASE let us know if you feel you are having difficulty and
need more help. If you are having problems or have other concerns, please
contact our Telecare Center (either at 713-794-8985 or toll-free at 1-
800-639-5137). These phone lines are staffed 24 hours a day, 7 days a week
======================= Additional info for patient ======================

============================== PCP follow-up =============================
Please follow-up with your primary care provider so they can continue managing
your chronic medical conditions.
